The artificial eyelash extension is dipped in a medical-grade adhesive and then placed over the natural eyelash. This process is repeated for up to 2 hours until the desired fullness of eyelashes is obtained.
The 90-degree rules simply mean that each eyelash extension should be placed at a 90-degree angle.
